DECISION DOCUMENTATION ASSESSMENT

Find where your decision record becomes hard to explain.

In about three minutes, assess one completed GMP decision and identify whether the record preserves the reasoning behind it—or leaves leadership to reconstruct it later.

QUESTION 1 OF 7

A completed decision under review

An executive reviews a high-consequence batch disposition record and finds the "Approved" status, e-signatures, and closed workflows are all present, but cannot find the justification for why specific sterile data deviations were accepted. In your organization, if an investigator asked for this justification today, what is the most likely scenario?

Choose one answer to begin. Your Executive Report appears on screen before any email request.

DECISION-RECORD EXPOSURE MAP

Where a completed decision becomes difficult to defend.

1. Reasoning gapThe record shows what was decided, not why.
2. Fragmentation gapThe decision basis is scattered across files, meetings, email, and memory.
3. Context gapAssumptions, constraints, alternatives, and uncertainty are not preserved.
4. Linkage gapThe evidence exists, but the record does not show how it led to the conclusion.
5. Authorization gapIt is unclear who held authority or what conditions they approved.

WHAT THE ASSESSMENT RESOLVES

See where the decision record is exposed.

01

■ Can the decision be defended from the record? Evidence

Establish whether the evidence, judgment, risk, and authorization remain connected to the decision record.

Resolves: whether the record can explain the decision without reconstruction.
02

▲ Will the reasoning have to be reconstructed? Judgment

Identify where explaining the decision would require email, meetings, systems, or institutional knowledge.

Resolves: where reconstruction burden is likely to accumulate.
03

● Where is the decision most exposed? Risk

Identify where consequential context depends on people or disconnected records.

Resolves: where decision-reconstruction exposure is concentrated.

These measures are reported independently. They do not create a composite compliance score or make a regulatory determination.

WHY IT MATTERS

The difference is what you can show, not just what you did.

Completed decisions become difficult to explain when the evidence, alternatives, risk judgment, and reasoning behind authorization are scattered across people, email, meetings, system exports, and disconnected records.

What most QMS records retain

  • Outcome
  • Status
  • Approval
  • Signature

What a defensible decision must retain

  • Evidence considered
  • Alternatives considered
  • Risk judgment at the time
  • Reasoning behind authorization
“Why was this decision authorized based on what was known at the time?”

If the record cannot answer that directly, the organization may have to reconstruct the answer later.

Assessment scope and boundaries

What the assessment examines

The DDA examines critical GMP decision areas, how decision evidence and context are preserved, whether risk judgment, alternatives, reasoning, and authorization remain available, and the effort required when historical context must be reconstructed.

Preventive and historical exposure are different problems. Preserving a record while evidence and reasoning are still available is different from organizing the available evidence after context has already been lost. ComplianceWorxs does not invent missing evidence, recreate undocumented facts, make a GMP determination, or predict an inspection outcome.
