The closure distinction

Action completion is not the same decision as effectiveness.

A CAPA can have completed tasks, training records, revised procedures, and a signed closure date without preserving why Quality concluded the corrective action was effective. The harder question is whether the evidence gave the original failure mode a fair chance to recur, and whether the remaining uncertainty was acceptable at closure.
